Lakeland MyChart Now Available to Community - Archived
(ST. JOSEPH) – As Lakeland HealthCare transitions to an electronic health records system, the community is encouraged to register for a free service called Lakeland MyChart. MyChart provides patients with a high-level summary of their medical chart as well as interactive features to keep in touch with physician offices. Those who register for a MyChart account before March 30 will be entered to win one of two prizes: an iPad2 or Amazon Kindle.
“Because the system is web-based, it ‘travels’ with a patient – MyChart can be accessed at home, at work, on vacation, or anywhere else with an internet connection,” said Norma Tirado, Vice President, Human Resources and Health Information Technology, Lakeland HealthCare.
Patients who visit a Lakeland HealthCare affiliate physician practice or who are discharged from a Lakeland hospital will receive a MyChart activation code on the “After Visit Summary Report.” The code is necessary to register at www.lakelandhealth.org/mychart. Community members may also request an activation code by calling (800) LAKELAND; a unique code will be mailed via U.S. Postal Mail. Similar to most secure websites, the created user ID and password are the keys to protecting the confidentiality of the information; each patient is responsible for remembering his or her own user ID and password and for keeping them confidential.

Request an appointment
Communicate with the doctor’s office
Access test results
Request prescription renewals
View recent clinic visits
Please note that MyChart should not be used for medical emergencies. Patients should contact their physician’s office directly if a situation requires immediate attention or dial 9-1-1 in the case of an emergency.
