Lakeland Offering New Tools for Patient Engagement
(ST. JOSEPH) – Lakeland HealthCare is the first healthcare provider in southwest Michigan to bring easy-to-understand, interactive, multi-modal health information to patients in our community through a new partnership with Chicago-based Emmi Solutions, LLC.
“Patients are at the heart of every care team at Lakeland,” said Vicki Buck, Patient Education Coordinator, Lakeland HealthCare. “Remembering or understanding medical instructions can be difficult and overwhelming. We don’t want those we serve to be in that situation.”
The interactive, multi-modal programs created by Emmi Solutions include web-based programs and interactive phone calls. The initiative — called myHealth Matters at Lakeland — uses simple language and visuals to help patients make sense of complex medical conditions, walk them through an upcoming procedure, or prepare them for a hospital stay. myHealth Matters allows providers to connect with patients and their families at their convenience — when they’re most ready to learn and retain information — and on the devices they already own, including computers, tablets, or smart phones. Each program is prescribed by a patient’s provider and encourages patients to take a more active role in their health.
“People who are well-informed and engaged in their care tend to feel more in control — this positively affects the entire healthcare experience and fosters better outcomes,” explained Buck.
The programs are narrated by an easily-understandable and empathetic voice along with images, text, and interactivity. The patented technology tracks the delivery and consumption of information, which will enable Lakeland Healthcare to measure the impact of patient engagement down to the individual patient level.
In addition to the interactive programs, Lakeland HealthCare is also using EmmiPrevent®. EmmiPrevent is a series of interactive call campaigns that drive patient action for preventive care. EmmiPrevent engages and educates patients on the importance of taking action and addresses gaps in care. The campaigns focus on encouraging patient behaviors such as scheduling screenings, immunizations, and other preventive appointments which also helps providers improve quality measures and patient satisfaction.
