The Laboratory Test Catalog provides collection instructions, specimen requirements, container information, patient instructions, CPT codes, and run schedules.

Please Note:   
All specimens submitted for testing must be accompanied by a valid physician order. Laboratory Service Order Sheet with Legend / Pathology Cytology Request with Legend.

All blood specimens must be collected according to a specific Order of Draw. Order of Draw for multiple tube collections.

All specimens must be labeled correctly. Specimen Labeling Instructions.
