Q: How can I have drug testing performed on my child?
A: Minor children, those who are under the age of 18 years:

• A physician must order the drug screen.
• The parent or guardian is required to be present at the time of collection in order to sign for the minor.

Q: My future employer requested that I have a drug test done prior to the start of my employment. What do I need to bring with me to your facility?
A: If your future employer requests a drug test prior to employment please call the Lakeland Scheduling Line at 269-556-2810 or 800-791-2810. Please bring the following information and/or paperwork with you:

1. The company name, address, contact person, and telephone number of the future employer.
2. A list of the drugs to be tested.
3. Any paperwork or forms you may have received from the future employer regarding your drug testing.
