How do I get started with hospice care?

An admissions coordinator will set up an appointment with a nurse and/or social worker to do an information visit.  During the information visit you will learn more about the hospice services available to you.  We will answer your questions and assist you with the required paperwork.  A nurse will then complete a physical assessment and can immediately review your health status with a hospice medical director to determine eligibility for hospice care.
