Join Speak for Yourself, Plan Your Care as we lead the conversation on advance care
planning during National Healthcare Decisions Day beginning the week of April 16.
In collaboration with various partners throughout southwest Michigan, we are working
together to offer the opportunity for individuals in the community to receive a detailed
conversation regarding their healthcare wishes.
