To schedule or re-schedule an appointment, save time and use your Lakeland myChart account or call our Niles office at (269) 684-5547 or St. Joseph office at (269) 983-3455. Please arrive 15 minutes prior to your scheduled appointment to allow time to check-in. We reserve the right to re-schedule an appointment if you are more than 15 minutes late. Please bring photo identification and your insurance card to your appointment.
Laboratory and Radiology Services
Lakeland Health provides imaging and laboratory services at multiple locations. Visit www.lakelandhealth.org/lab to choose the location most convenient for you. Appointments can be scheduled by calling (800) 791-2810. Please be advised that because these services are performed outside of the office, you will receive a separate bill in addition to your office visit. Lakeland facilities use
electronic medical records, so you will not need any paperwork to take with you to these appointments.
Before Your Appointment
A team of medical providers will work with you to help you meet your health goals. You, your doctor, and the rest of the care team will work together to make your visit as helpful to you as possible. Here are a few things you can do to ensure your appointment is a success:
- Bring all of your current medications in their original containers to your appointment – including over-the-counter medications, prescriptions, and vitamins you take on a regular basis.
- Make a list of your health questions, with the most important at the top. We will answer as many as we can during your appointment, but may need to schedule another appointment to answer all of your questions.
- You’re welcome to bring a family member or trusted friend with you to your appointment
