Please Note:
All specimens submitted for testing must be accompanied by a valid physician order.
All blood specimens must be collected according to a specific Order of Draw: Order of Draw for Laboratory Collections
All specimens must be labeled correctly: Specimen Labeling Instructions.

Effective November 3rd, 2020, the CDC questionnaire must be filled out with every COVID -19 order not entered in the Spectrum Health Lakeland EHR. The form can be found here.
