Preparing Your Taxes: 1095-C Forms

This is the second year that the Affordable Care Act (ACA) requires employers to send form 1095-C to those who were offered and/or enrolled in Lakeland’s health plan in 2016.  The purpose of this form is to document the health insurance you were offered/enrolled in during 2016. The 1095-C form should be kept in the same manner as you retain your annual tax documents, forms, and other paperwork such as your W-2.

The due date for furnishing the 1095-C form has been extended by the IRS to March 2, 2017. You will receive your form in the mail by March 2, 2017. At this time, there is not an option to print the document from Employee Self Service. 

If you file your income taxes before you receive your 1095-C form, you will NOT be required to amend your tax return once you receive your form.
