With the health needs of our older community members in mind, Spectrum Health Lakeland has formed the Senior Services Consortium. This group brings together leaders from a number of different entities that serve older patients in southwest Michigan, including hospitals, nursing homes, homecare services, and retirement communities.

Our goals:  

  • Build a coalition of providers to provide the best possible care for older community members, whether they are in the hospital, the doctor’s office, a nursing home, or anywhere else
  • Raise the bar on quality, safety, and service at all organizations serving southwest Michigan seniors by working together on opportunities for improvement

Key Initiatives
Coordinated Care Through EpicCare Link
As patients age, it’s more important than ever for their entire care team—primary care physicians, specialists, and other providers—to work together to provide consistent, coordinated care. The consortium has made that possible by giving providers secure access to view their patients’ electronic medical records through EpicCare Link. For patients, this means that their entire care team can collaborate more quickly and effectively.

Caregiver Education for Healthier Seniors
One way to measure the consortium’s success is by how effective it is at keeping seniors healthy and out of the hospital. Lakeland has made strides toward this goal by educating staff at participating facilities about management and prevention of congestive heart failure and other medical conditions that are some of the most common causes for hospital admissions.

Collaborating for Emergency Preparation
If a natural disaster struck or other emergency struck in southwest Michigan, it would be more important than ever for community organizations to work together to keep our community safe. The consortium has plans in place for how to care for older adults in various emergency scenarios, and participating organizations regularly conduct drills to make sure everyone is ready.
