How to refer a Patient

Admitting your patient to Lakeland

The guidelines below are intended to make it easy and efficient for you to directly admit a patient or schedule a patient for admission. The members of our admitting department are happy to help you or your office staff arrange for the care of your patients at our facilities. 

For Direct Admission

Step One
Call Transfer Facilitator (866) 574-7227 or (269) 556-7178. Be prepared to give patient information:

  • Patient name
  • Date of birth
  • Diagnosis
  • Any Special needs (telemetry, isolation etc).

Transfer facilitator will contact the Hospitalist/Intensivist.

Step Two
The Hospitalist/Intensivist will call referring physician to accept admission and discuss patient.

Step Three
Transfer Facilitator will call the referral source with bed assignment.

Step Four
If the patient is arriving at the hospital during the hours of:

  • 5:30 a.m. to 7:30 p.m. - please direct patient to go through main entrance of the hospital to the admitting office.
  • 7:30 p.m. to 5:30 a.m. - please direct patient to go to the ER triage desk for assistance with being directly admitted.
