The Senior Center Nursing Program
The constantly changing scope of healthcare can be confusing to anyone, but for our aging population it can be even more daunting knowing where to begin. To help connect seniors with the care and education they need, Lakeland Health and Population Health teamed up with Senior Centers across Southwest Michigan.
Each participating Center is staffed by one of our Senior Center Nurses. They coordinate important health screenings for blood pressure, diabetes testing, and cardiovascular health. In addition, Senior Center Nurses counsel and advise those dealing with lifestyle changes or health concerns, and can refer seniors for physician care, when necessary. They also act as a liaison - visiting senior center members in hospitals, nursing homes, and those who are home-bound.
Senior Center Nurses are committed to serving as an advocate for the aging population so they receive quality care and excellent service.
