Deanna Rucano, FNP
Deanna has been part of the Lakeland family for more than half her life! Before serving her patients at Lakeland Primary Care, St. Joseph, the New Buffalo native worked as a surgical registered nurse at Lakeland Hospital Niles, where she helped our surgical team and patients prepare for and recover from surgery. She also has long term acute care experience from the former Lakeland Specialty Hospital in Berrien Center. Deanna recently completed her Master of Science Degree in Nursing at Indiana University in South Bend. She is a member of the American Association of Nurse Practitioners.
Jay Shah, MD
Dr. Shah always knew he wanted a career in medicine. He truly enjoys the way family medicine allows him to form personal relationships with people of all ages. Dr. Shah earned his medical degree from Rosalind Franklin University of Medicine and Science in Chicago. He completed his residency in family medicine at Advocate Lutheran General Hospital in Park Ridge, Illinois, where he learned and practiced under the Patient Centered Medical Home model. In 2010, Dr. Shah earned the Family Medicine Clerkship Award. He is a member of the American Academy of Family Physicians. He also serves as Medical Director of the Lakeland Hospital Watervliet clinics.
Donald Wolfram, MD
After years spent in emergency medicine and serving the medical needs of our military members with Veterans Affairs, Dr. Wolfram returned to Michiana to fulfill his dream of working in family medicine. Dr. Wolfram earned his Bachelor of Biology degree from the University of Kansas before pursuing his medical degree from the University of Kansas School of Medicine. He went on to complete his Family Medicine residency at Saint Joseph Regional Medical Center in Mishawaka, Indiana. Dr. Wolfram served in the United States Marine Corps, Infantry from 1978-1981, and has also worked as a medical missionary with his wife, Roxanne.
Roxanne Wolfram, DNP
Roxanne shares her love of family medicine with her husband, Donald. She has decades of experience caring for heart patients in a Cardiothoracic recovery unit in South Bend, Indiana, as a critical care travel nurse in Nevada and California, and has also worked with surgical and trauma patients at Butterworth Hospital in Grand Rapids, Michigan. Roxanne spent time surrounded by healthcare academics when she was a Clinical Assistant Professor and Graduate Program Director at Indiana University in South Bend, and faculty member at Ball State University and Bethel College in Mishawaka, Indiana. She received both her Doctor of Nursing Practice and Master of Science in Nursing from Valparaiso University in Valparaiso, Indiana, after completing her Bachelor of Science in Nursing from Nazareth College in Kalamazoo, Michigan.
