Do You Have a Plan?

During the month of April, everyone is encouraged to take the steps needed to make an advance directive and ensure their voice is heard in the event of a medical emergency. Southwest Michigan’s program Speak for Yourself, Plan Your Care is a collaboration of community partners that offers the opportunity for individuals to discuss their healthcare wishes with certified advance care planning facilitators. To start the conversation, begin by picking up free advance directive information from one of the following locations:

  • Benton Harbor: Lakeland Care Network, Strong Women of Faith
  • Bridgman: Southwestern Medical Clinic
  • Buchanan: Southwestern Medical Clinic
  • Cassopolis: Cassopolis Family Clinic
  • Coloma: Coloma Medical Center
  • Hartford: Stagg Medical Center
  • Niles: Lakeland Family Medicine, Niles District Library, Southwestern Medical Clinic, West Woods of Niles
  • St. Joseph: Berecz and Associates, Area Agency on Aging, Region IV, Lakeside Healthcare Specialists, PACE of Southwest Michigan, Southwestern Medical Clinic, Caring Circle
  • Stevensville: Southwestern Medical Clinic
  • Watervliet: Community Medical Center

After reviewing the information, take these steps to put your advance directive in place:

  • Schedule a conversation with a facilitator to discuss what’s most important to you
  • If you already have an advance directive, review it with a facilitator to ensure it still reflects your wishes
  • When you are comfortable with your advance directive, provide copies to your advocates and family so they are prepared to be your voice if something happens
  • Provide a copy of your advance directive to the admissions department of your local hospital for placement in your electronic medical chart
