Team Members to Receive Form 1095-C

The Affordable Care Act (ACA) requires employers to send Form 1095-C to those who were offered or enrolled in the Lakeland health plan. This form documents the health insurance you were offered or enrolled in during 2018 and should be kept with other annual tax documents and paperwork such as your W-2.
The IRS due date for mailing the 1095-C is March 4. You will receive your form in the mail around March 4. At this time, there is not an option to print the document from Workday.
If you file your income taxes before you receive your 1095-C, you will not be required to amend your tax return once you receive the form.
