Comprehensive Primary Care Plus
Our Southwestern Medical Clinic Adult and Family Medicine practices are participating in the nation’s largest ever program to improve primary care. Comprehensive Primary Care Plus, or CPC+, offers primary care practices greater flexibility in caring for their patients the way they think will deliver the best outcomes.
