Appointment Information

Your First Appointment

Please call the Southwestern practice of your choice to schedule your first appointment.  Be sure to tell them you are a new patient and have your insurance information ready.

Please bring the following to your first appointment:

  • Immunization record
  • Medical records from your previous physician
  • List of medications the patient is taking

Returning Patients

To request your appointment, use your Lakeland myChart account, or call your chosen practice. Our helpful and friendly front desk associates help provide smooth scheduling and little to no wait times. We strive to make sure your visit with us is as pleasant as possible, and all your questions are answered.

We reserve the right to re-schedule an appointment if you are more than 15 minutes late. Please bring your insurance card and photo ID to your appointment.

We think it’s important to offer appointments in a timely manner so you can receive care when you need it. Call the practice as soon as possible if you need to cancel an appointment.

Healthcare Collaboration Policy

Providing less than 24 hours’ notice may be documented as a “no show.” We understand sometimes unavoidable emergencies and busy schedules interfere with scheduled appointments. Three “no shows,” or missed appointments, in a 12-month period, may result in dismissal from the practice. Any incident of abusive or rude behavior toward physicians or staff members may also result in dismissal.

Patients dismissed from our practice receive 30 days of emergency care services. Medical records are transferred to a new doctor upon patient request.
