Patient Information:
• Specimen must be collected between the 15h and 22nd week of gestation for proper risk analysis (optimal time is 16-18 weeks)
• Collect sample prior to amniocentesis
• Obtain the following information and send with sample: (Information sheet (pdf) above - can be printed)
» Expected date of delivery (EDD)
» Number of fetuses in current pregnancy (single vs. multiple)
» Mother's race
» Mother's status of insulin-dependent diabetes
» Mother's weight in pounds
» Mother's exact date of birth (mm/dd/yy)
» Is this a repeat sample? (yes/no)

NOTE:
• Pregnancies with multiple fetuses require the date of the last ultrasound and the gestational age of each fetus at the time of the ultrasound
• Pregnancies of >2 fetuses cannot be assessed
